In preparation for a visitation, schools are required to complete the applicable online forms.

 

All forms and supporting documentation must be submitted two weeks prior to the visitation.

You can "save" your progress on each form and revisit it later for further editing until the form is submitted. The save button is located at the bottom of each form. Saving a form generates a unique link, which can be shared with others at the school if you require their assistance in completing the form.
